Effective 2026 · This policy explains how Perrenia Wellness collects, uses, stores, and shares your information — including SMS opt-in data — when you use perreniacare.com or coordinate care through our program.

What we collect

Information you give us when you book or message — name, contact, health intake answers, payment, and (with your explicit consent) mobile number for SMS coordination reminders.

How we use it

To coordinate your care with independent licensed partner providers, schedule consultations, deliver lab review, and (with your consent) send program-related SMS reminders.

What we never do

Sell or rent your information. We do not share your phone number or SMS opt-in data with third parties or affiliates for marketing purposes.

1. Who we are

Perrenia Wellness is an online care-coordination program operated by Perrenia Wellness Care LLC. Our website is perreniacare.com. We coordinate virtual consultations, lab review, and lifestyle-supplement plans with independent, separately licensed partner providers and partner laboratories. We do not practice medicine, and we do not prescribe, dispense, ship, fulfill, or take title to any medication.

2. Information we collect

We collect information in three ways: (a) you give it to us directly, (b) it is generated automatically as you use the site, and (c) it is provided by independent licensed partner providers who coordinate your care through our program.

  • Account & intake information — name, date of birth, contact details (email, mobile number, mailing address), and the answers you provide on our intake forms.
  • Health-relevant context — goals, current medications, allergies, recent labs (if uploaded), lifestyle and medical history you choose to share.
  • Payment information — coordination-fee billing details, processed by a PCI-compliant payment processor; we do not store full payment-card numbers.
  • Communications — messages you send us by email, SMS, web form, or phone; voicemail transcripts where we have your consent or where required by law.
  • Mobile & SMS opt-in data — your explicit opt-in (form checkbox, signed intake, or YES reply to a confirmation prompt), consent timestamp, the program the opt-in applies to, and a record of subsequent opt-in/opt-out events.
  • Technical information — IP address, browser, device, pages viewed, referrer, and timestamps captured automatically by our hosting, analytics, and security infrastructure.

3. How we use your information

We use the information we collect to operate, maintain, and improve the care-coordination program, including: matching you with a partner provider; scheduling consultations; routing lab orders and lab results; sending program-related messages (email and, where you have opted in, SMS); customer support; billing and accounting; detecting fraud and abuse; and meeting our legal and compliance obligations.

We do not use your information for advertising networks, ad-targeting auctions, cross-context behavioral advertising, or any sale-of-personal-information regime.

4. SMS program — full terms

We do not sell, rent, or share your mobile phone number or SMS opt-in data with third parties or affiliates for marketing purposes. Mobile opt-in data and consent records are kept inside Perrenia Wellness and the SMS-vendor account we use to send program messages, and they are used only to deliver and support the Perrenia Care Coordination SMS program.
  • How we collect your number / consent. You provide your mobile number through a Perrenia intake form, the booking flow on perreniacare.com, or by replying YES to a confirmation message we send after a verbal request. Each opt-in records the source (form, page URL, timestamp) and a record of the consent language presented.
  • What we send. Recurring SMS messages related to your coordination: appointment confirmations and reminders, lab-result arrival notifications, prescription-status updates from your matched pharmacy, follow-up prompts from your care team, and one-time program announcements. No marketing blast campaigns, no third-party offers.
  • Message frequency. Varies by member activity; typically 1–6 messages per month.
  • Cost. Message and data rates may apply per your mobile carrier plan. Perrenia does not charge for SMS itself.
  • Opt out at any time. Reply STOP (or STOPALL, UNSUBSCRIBE, CANCEL, END, QUIT) to any program message. We will honor the opt-out within one business day and send a one-time opt-out confirmation.
  • Help. Reply HELP to any program message, email [email protected], or call (910) 239-3100.
  • Supported carriers. Major U.S. carriers including AT&T, T-Mobile, Verizon, UScellular, and most MVNOs. Carriers are not liable for delayed or undelivered messages.
  • Privacy of SMS data. Mobile opt-in data, consent, and message history are used solely for delivering the Perrenia Care Coordination SMS program. We do not transfer this data to any third party for marketing purposes.

5. Cookies and similar technologies

We use a minimal set of first-party cookies and similar storage to keep the site working: remembering whether you have closed our cookie banner, remembering your form-progress state during intake, and protecting against abuse. We do not deploy cross-site advertising cookies. Where the law (e.g., EEA/UK visitors) requires opt-in consent for non-essential cookies, we prompt before any non-essential storage is loaded.

6. How we share information

We share information only as needed to operate the program, with parties under confidentiality and data-protection obligations:

  • Independent licensed partner providers — clinical information necessary to coordinate your consult.
  • Partner laboratories — to process lab orders you have authorized.
  • Pharmacy partners (e.g., Qualiphy) — only if and when an authorized prescription is written by your matched provider, and only the minimum needed to dispense.
  • Service vendors — hosting, payment processing, SMS-vendor, secure email, and audit-log providers, all under contract.
  • Legal & safety — when we believe in good faith that disclosure is necessary to comply with law, court order, or to prevent imminent harm.

We do not sell, rent, or trade personal information for consideration. We do not share SMS opt-in data with third parties or affiliates for marketing purposes.

7. Data retention

We retain coordination records for the period required by applicable healthcare, tax, and consumer-protection law (typically seven years for clinical/billing records) and then de-identify or delete them. SMS opt-in/opt-out events are retained for at least four years to demonstrate compliance with carrier and FTC messaging rules.

8. Security

We use administrative, technical, and physical safeguards designed for healthcare-coordination data: HTTPS in transit, encryption of sensitive fields at rest, access limited to staff and vendors on a need-to-know basis, audit logging, and incident-response procedures consistent with HIPAA Security Rule expectations for a care-coordination program. No system is perfectly secure; we notify affected members of any breach involving their personal information consistent with applicable state and federal law.

9. Your choices and rights

  • Access & correction — request a copy of your coordination record and correct inaccurate information.
  • Opt out of SMS — reply STOP, or email [email protected] and we will process your request within one business day.
  • Marketing email opt out — every program email contains an unsubscribe link.
  • State-privacy rights — residents of jurisdictions including California (CCPA/CPRA), Colorado, Connecticut, Virginia, Utah, Texas, Oregon, Montana, Iowa, Tennessee, Indiana, Kentucky, Maryland, Minnesota, New Hampshire, New Jersey, Rhode Island, Delaware, and others may have additional rights to know, delete, correct, or limit use of personal information. We honor these requests consistent with our legal obligations; we will not discriminate against you for exercising them.

10. Children

The Perrenia Wellness program is for adults. We do not knowingly collect personal information from anyone under 18. If you believe we have collected information from a minor, contact [email protected] and we will delete it.

11. Service area and international visitors

Coordination services are available in 48 U.S. states. We cannot currently coordinate care for residents of Alabama, Mississippi, or Missouri. Our website may be viewed from outside the United States; if you choose to engage the program, your information will be processed in the United States under U.S. law.

12. Changes to this policy

We will post material changes on this page and update the "Last updated" date above. For changes that expand how we use previously collected information, we will give you reasonable advance notice (typically by email or in-account message) so you can opt out before the change takes effect.
